MoonRise Community Outreach is excited to announce they will be going out into the community to hand out gift cards/donations/ect to those who are living their normal life in society to raise awareness that an act of kindness can spread into a systemic change of humanity as a whole. Starting August 1st, 2026-August 31st, 2026.
We offer and deliver requests of donated items to those who for whatever reason may not have the ability to have transportation of any kind. *We kindly ask that if you do not have access to transportation of any kind and are needing items to please reach out to our intake coordinator or if you are a client that you reach out to your assigned case manager.
1:1 Services
We are a dedicated team committed to crafting exceptional experiences. Our focus lies in providing reliable and innovative solutions, driven by a deep respect for quality and a genuine desire to exceed expectations. We offer case management to support with behavioral, housing, general assistance, applications, and much more! We offer free 1:1 support for those needing an extra hand during this journey of their life. 1:1 support is 60 minutes and can be in-person or Telehealth weekly. We offer support in mental health, substance use disorders, and dual.
IOP
From modest beginnings, we've grown through unwavering dedication and a commitment to continuous improvement. With intensive outpatient through us. You have the control of what fits your scheduling needs the most. To stay enrolled in IOP clients must maintain the following regulations and guidelines:
-Minimum of 9 hours a week, maximum or 19 hours a week.
-Must be able to attend group session every Wednesday evening at 7:00pm
-Must commit to a minimum of 30 days of IOP
-Must attend and have your meeting log signed showing a minimum of outside meetings of 3 a week. Log sheets are turned into your case manager every Monday unless otherwise approved beforehand.
IOP is great for those needing inpatient care but the flexibility of outpatient. IOP is Monday-Friday starting at 5:00PM ending 9:00PM. With mandated group on Wednesday's 7:00PM-8:30PM. You can choose your hours depending on availability during any of the IOP hours on those days to make your custom schedule.
